新型农村合作医疗制度：实践与创新

 
摘  要

自2003年新型农村合作医疗试点工作开展以来，在为部分农民提供最基本的医疗保障方面，新型农村合作医疗已经取得了初步的成效。但是，在新型农村合作医疗制度全面推进中，还存在着一些突出问题：农民参合积极性还不高，管理体制不健全，尚未适应工作需要，合作医疗基金筹资成本大，补偿标准不尽合理，补偿能力有限；医疗监管难度系数较大；立法滞后，缺乏合理的人才机制等等。新型农村合作医疗制度的规范与完善，需要从几个方面着手：加强管理机构和管理服务能力建设；扩大并进一步探索新的筹资渠道，强化政府责任，改善与外部的关系，形成良好的互动局面；尽快建立城乡一体的医疗保障制度和管理制度；强化新型农村合作医疗监管，提高管理效率；加快相关立法，确保新型农村医疗保障制度政策的稳定性；加强新农合人力资源建设，重视乡镇卫生院人才培养。要完善新型农村合作医疗工作，必须在筹资机制、监督机制、保障机制等方面进行制度创新。

Abstract

Since 2003 the work of new type of rural cooperative medical scheme was carried out, the new type of rural cooperative medical care has achieved preliminary results in the aspect of providing some farmers with the most basic medical security.. However, in the full forward of promoting the new rural cooperative medical care system, there also exists a number of outstanding problems: Farmers’ participating enthusiasm is not high; management system is not perfect, not yet adapted to work needs; the costs of cooperative medical fund-raising is high, the compensation standers is unreasonable, the compensation capacity is limited; degree of difficulty of the medical supervision is large; lagging legislative, lack of mechanisms for rational talents and Etc. In the future, the specification and perfection of the new rural cooperative medical system required to be commenced in several aspects: strengthening capacity building of the management institutions and management services; expanding and further exploring new financing channels, intensifying the government’s responsibility for improving the outside relations and forming a good interaction situation; establishing urban-rural integration medical security and management systems as quickly as possible; strengthening  supervision of the new rural cooperative medical and improving management efficiency; seeding up the relevant legislation and ensuring policies stability of the new rural medical security system; reinforcing new rural cooperative development of human resources; attaching importance to personnel training of the township hospital. In order to improve the new rural cooperative medical work, the government should conduct system innovation in financing mechanisms, monitoring mechanisms, security mechanisms and other aspects.
