我国农村医疗保障制度改革存在的问题及对策
摘 要
家庭联产承包制实施以来，我国农村医疗保障制度面临困境，大部分农村居民自费医疗，一些地区农村因病致贫、因病返贫问题突出。近年来以合作医疗为核心的农村医疗保障制度改革取得了一定成绩，但也存在资金筹集难，模式单一化等问题。我国各地区经济收入、医疗消费、文化观念差异相当大，本文建议不同经济发展水平地区因地制宜地选择和发展多种模式的农村医疗保障制度：东部地区建立以农村住院保险为主的多种医疗保险模式；中部地区探索建立以新型农村合作医疗为主体，配合多种形式的补充医疗保险制度；西部经济条件较差的地区建立以医疗救助为主的模式，西部经济发展较好的地区可以学习中部地区实行新型农村合作医疗制度，在此基础上发展多种形式的补充医疗保险。并同时落实政府在医疗保障中的责任，做好农村贫困人口的医疗救助，推进农村医疗卫生机构改革。

THE EXISTING PROBLEMS OF CHINA'S RURAL MEDICAL SECURITY SYSTEM REFORM AND COUNTERMEASURES
ABSTRACT
Since the implementation of family-contract responsibility system, China's rural medical security system faces many difficulties, a majority of rural inhabitants pay the medical costs themselves. Some problems such as becoming poor and returning to poverty caused by diseases are outstanding in some rural areas. In recent years the rural medical security system reform which has cooperative medical care as its core has made certain achievements, but there are also some problems. For example, the difficulty to raise funds, a simply model, and so on. This paper proposes different rural medical security system be set up according to the great differences in economic revenue、medical consumption、cultural values in different regions of our county. The proposals include: the establishment of multi-kind medical insurance models in eastern region while puts rural hospitalization insurance first; new rural cooperative medical modes accomplished by other medical insurance system should be set up in middle region; medical assistance models should exist in areas in poor economic conditions in western region. And the western areas of economic development better, can learn from the central region to implement the new rural cooperative medical system, on the basis of the development of a variety of supplementary medical insurance.At the same time, implementing the government's responsibility in medical protection, doing things right in the rural impoverished population's medical assistance, and promoting the rural medical and health institution.
