摘  要：秘书是领导的助手，要帮领导收集信息，而在21世纪的信息时代，问卷调查是获取信息的重要手段之一，要想有一份好的问卷，问卷设计很重要。能否根据实际情况设计出一份完美的问卷, 在很大程度上决定了调查问卷的回收率、有效率、以及回答的质量, 以致一项调查的成败。但是, 设计一份完善的问卷并非一件轻而易举的事情, 问卷设计人员除了要具备多方面的知识外, 还需要掌握一定的技术。这种技术涉及问卷的设计程序，设计原则，至问卷问题的语气措辞、排列，答案的全面等等方面。本文从多方面论述了秘书设计一份高质量问卷所需注意的问题。

On the design of questionnaire in a questionnaire survey
 
Abstract: Secretary is an assistant who need to help leaders gather information.In the 21st century information age, the questionnaire survey is an important means of access to information, to have a good design is very important. Can design a perfect questionnaire aaccording to the actual situation ,to a large extent,determine the recovery rate, efficiency, and quality of answers of the questionnaire and the success or failure of a survey. However, the design of a comprehensive questionnaire is not a simple matter, The questionnaire designers not only need have a wide range of knowledge, but also certain technology. This technique involves the questionnaire design process, design principles, to the tone of the wording of the questionnaire questions, arrangement, comprehensive answer. This article discusses the requires that a secretary design a high-quality questionnair should take in mind from many ways.
